Student Organization Shirt/Merch Ordering

Student Organizations that wish to order shirts or other merchandise should complete this form. The Office of Student Activities must approve this form before the Org finalizes an order. OSA pays up to $15 per member. If items cost more than $15, members must pay the difference.
